Parent/Guardian Waiver Form
Students must have the waiver form completed by a parent or guardian.
Every care and attention will be given to the well being and safety of participants in the MusiCamp Alberta. All participants will be supervised both day and night. A high standard of conduct will be expected at all times. Students may not leave the college grounds unsupervised.
MusiCamp Alberta and its staff will be absolved from any claims as a result of off-campus activities. Any unacceptable behavior will be dealt with by the Camp Director and violators will be sent home with no refund of registration.
Supervisors cannot be held responsible for any accidents that may occur.
Please note: the use of cell phones or internet devices is prohibited during rehearsals, classes and organized recreation. We reserve the right to confiscate cell phones/Internet devices.
Medications must be sent in original pharmacy bottle, correctly labeled with doctor's and student's name, and instructions. All medications (including aspirin, acetaminophen and ibuprofen) must be turned in at check-in. Medications will be administered by camp staff.
Emergency hospital and clinical services are readily available to MusiCamp Alberta. Supervisors are on 24-hour duty at the workshop. If a student requires a prescription and does not have sufficient pocket money for same, parent/guardian will be advised and be requested to pay.
